
Healthy Families works in partnership with families to provide information and support that nurtures healthy development of children during the early years of life through home visitation services. We offer both pre-natal and parenting support for families with children ages 0-6. Our enhanced home visitation also supports engagement with siblings within the home up to age 13.

We encourage families to have voice and choice in the planning of home visits. We work closely with community partners to ensure a holistic approach to meeting needs that you identify for your family and to build awareness of community supports.
As a further extension our Parent Child Assistance Program (PCAP) program offers specialized, holistic support during and after pregnancy for those experiencing difficulties with substance use and other health and social issues. Home visitors work directly with participants and families offering personalized services for up to 3 years. You may be eligible to receive services if you are pregnant or up to 6 months postpartum, self-report alcohol and/or drug use during pregnancy, and are ineffectively engaged with community.
Anyone can refer to the Healthy Families program including: Self, Parents, Doctors, Public Health Nurses, Social Workers, Teachers, Prenatal Instructors, Community Agencies.
